A Case of Jdiopathic Portal Hypertension




Abstract
Idiopathic portal hypertensionis a relatively uncommon disorder characterized by overt splenomegaly with pancytopenia, esophageal varices, portal hypertension and mild abnormalities in liver function tests. A fifteen year old boy presented with
hematemesis and abdominal mass as the chief complaints. Pancytopenia was present and the liver function was normal. Abdominal ultrasonography demonstratedhuge splenomegaly and centripetal portal flow without evidence of thrombosis The
esophagogram
showed severe esophageal varices involving lowere two-thirds and splenoportogra;hy disclosed well developed periportal collaterals without intraportal obstruction. The splenic wedge pressure was 290 mm H2O. A intraoperative liver biopsy showed
periportal fibrosis without cirrhotic change and many collateral venules. The patient underwent splenectomy, gastric devascularization and conventional splenorenal shunt. Esophagography done after operation demonstrated much decreased size of
esophageal
varices.
